Support Coordination Level 3 is designed for individuals with significant and complex support needs who require intensive coordination and assistance to navigate the NDIS and access appropriate services. Our highly skilled support coordinators work collaboratively with clients, their families, and other stakeholders to develop holistic support plans that address all aspects of their lives. We provide comprehensive support coordination services, including assistance with plan development, implementation, and review, as well as ongoing monitoring and advocacy. Our coordinators have extensive experience working with individuals with complex needs, including those with multiple disabilities, mental health challenges, or other barriers to participation. Our goal is to ensure that individuals receive the highest quality of support, enabling them to live meaningful, fulfilling lives and participate fully in their communities.

Our coordinators conduct a thorough assessment of the individual’s needs, preferences, and goals to develop a holistic support plan that addresses all aspects of their life, including health, housing, education, employment, and social participation.

 

Specialised Plan Development:

We work closely with the individual and their support network to develop a specialised support plan that outlines the specific supports and services needed to address their complex needs and achieve their goals.

 

Intensive Plan Implementation:

Our team provides hands-on assistance with implementing the support plan, including coordinating services, arranging appointments, and facilitating communication between service providers and the individual. We ensure that supports are effectively utilized and aligned with the individual’s goals and preferences.

Ongoing Monitoring and Review:

We conduct regular reviews of the support plan to ensure that it remains effective and responsive to the individual’s changing needs and circumstances. We make adjustments as necessary to optimize outcomes and well-being.

 

Advocacy and Crisis Support:

Our coordinators advocate on behalf of individuals to ensure their needs and preferences are effectively communicated and addressed by service providers, government agencies, and other stakeholders. We provide crisis support in times of emergency or crisis, working collaboratively with other support services and agencies to ensure the individual’s safety and well-being
